We divide the claims into 2 categories: Medical claim & Major claim

(I) Medical claim: Hospitalization/Accident Hospitalization/Accident Outpatient Reimbursement

• Policy Contract
• Claim Form
• ID copy
• Medical Record of the insured issue Grade 11 or above Hospital Appointed by the National Health Department Of China
• Clinic, Emergency and Hospital Original Receipts for medical expenses
• Prescription list and hospitalization discharge summary
• Disability Assessment Form
• any other proof or information to substantiate the nature, cause and of
  the insured loss

(II) Major Claim including: MAT ,Death Claim, Severe Disease Claim and Accident Disability Death etc.

• Policy Contract
• Claim Form
• ID copy
• Medical materials
• Medical birth certification (* MAT claim)
• Death Certificate of insured issued by Police or Grade II or above Hospital appointed by the National Health Department in China (* death claim)
• Cremation certification(*death claim)
• Cancellation proof of insured’s household record(*death claim)
• Personal and Household identity of beneficiary (* death claim)
• Traffic accident certification(*traffic accident claim)
• Disability Assessment Form(*disability claim)
• any other proof or information to substantiate the nature, cause and of the insured loss
