of policy change
Procedure
Change corresponding address/telephone number(Time
limit:any time):
If your corresponding address or telephone number has been changed,
you should notify us in written as soon as possible. We will amend
the computer record accordingly and all future correspondences will
deliver to the new address.
Change insured's information (Time limit: any
time when the policy is in force):
You are required to complete the 'Restatement/Service Application
Form'to rectify your personal information including name,age,identity
card number,occupation,height,weight and personal health record
etc.You also need to provide relevant proof to substantiate.
Change beneficiary(Time limit:any time when
the policy is in force):
You have the right to change the beneficiary designated.You should
fill in the 'Restatement/ Service Application Form'and submit the
policy.
Change signature specimen of insured/policyowner(Time
limit:any time):
You need to complete PartI&II of the 'Reinstatement/Service
Application Form' to show your old and new signature specimen. In
addition, you need to provide copy of identity card and policy.
Change pay mode(Time limit: one month before
the old premium pay to date):
You can choose among monthly, semi-annual and annual pay mode. You
need to complete Part I of the 'Reinstatement/Service Application
Form',sign on it,submit policy and pay corresponding amount of renewal
premium. If you choose monthly pay mode, you should pay two months
premium and give us your bank account number and complete the 'Direct
Debit Authorization Form'.
Reduce sum insured of basic plan(Time limit:
between premium pay to date to grace period):
You can choose to reduce the sum assured after we issue the policy.
You need to complete Part I of the 'Reinstatement/Service Application
Form',sign on it and submit the policy.
Add rider(Time limit:policy anniversary date
to grace period):
You can request to add any rider attached to the plan.You need to
complete Part I&II of the 'Reinstatement/Service Applicant Form'
for this change.If need medical check,the check fee is paid by the
customer.
Cancel rider(Time limit: premium pay to date):
You can complete Part I&II of the 'Reinstatement/Service Applicant
Form' and submit policy to cancel rider.
Apply/Change bank account for auto-pay (Time
limit: one month before premium pay to date or earlier):
You can submit your renewal premium through auto-pay service. No
matter you are the first time to choose this service or need to
change the bank account number that you used, you need to complete
the 'Direct Debit Authorization Form'.
Re-issue policy contract (Time limit: any time
when the policy is in force):
If your original policy contract has been lost or damaged, you can
request us to reissue policy contract to you. You need to give us
written notification and submit 10 yuan for service charge to make
that change.
Convert term policy(Time limit:refer to the
contract):
If your policy is a term policy, you can change your policy to a
whole life coverage in accordance to the term and condition in your
policy. You need to complete the 'Reinstatement/Service Application
Form' for this change.
Policy loan(Time limit: any time when the policy
is in force):
You can apply policy loan from the cash value of your policy and
this will not affect the validity of your policy. The maximum amount
of loan should be up 70% of the net cash value under the policy.
You need to complete the "Reinstatement/Service Application Form'.
Surrender policy (Time limit: any time when
the policy is in force):
You can complete the 'Surrender Form' to surrender your policy.
We will calculate the surrender value and return to you in accordance
to the contract provision.
Cool off policy(Time limit: ten days after
the reception date):
You can request to cool off your policy within the period specified
by the contract in written.
Fund withdrawal(Time limit: any time when the
policy is in force):
You can complete the "Reinstatement/Service Application Form', tell
us your withdrawal units, the minimum withdrawal amount is 500 yuan,
and your account balance must be large than 1000 yuan.
Fund switching (Time limit: any time when the
policy is in force):
You can complete the "Reinstatement/Service Application Form' to
make that change.
Premium holiday (Time limit: one month before
premium pay to date, you can end the holiday at any time):
You can complete the "Reinstatement/Service Application Form' to
make that change, but your account balance should be large than
3000 yuan.
